1. Complete the Business Membership Application.

2. Please complete the application thoroughly and note the following:

– Designate “New Membership” at the top of the form.

– Under the Business Information Section, list an address to mail all account information.

– Ensure that the application is signed by all Local officers.

3. Provide a copy of the NATCA Local Update Worksheet.

4. Provide a copy of the Local’s By-Laws.

5. Please include a copy of all signers Driver’s License.

6. Provide a copy of the NATCA EFT Form and a voided check of the account to receive the funds.
